How to Use This


Copy and Paste each section into an e-mail and provide the requested information. If you have an Excel spreadsheet of the information, then you can attach those but please mention that on the appropriate item.

  • Date of anticipated return to US, if any

Fees will include courier transport (FedEx, DHL, or other as you prefer and time allows) of finished returns to you.

Please note that if all required information is not received prior to May 15, we reserve the right to file an automatic extension for you. NOTE: Any taxes due must be paid by April 15, even if your return is not due until June 15 or later.
